Direct Primary Care Memberships
Fees
All fees for primary care services will be billed using a direct primary care model.
Insurance
Insurance including Medicare, Medicaid and private insurance will not be billed for clinic provided services.
Members
Patients must be “members” to receive treatment for acute or chronic illness, as well as for preventative exams such as physicals.
Non-Members
Some services (as listed below) will be available to “non-members” on using a fee based system, however this will not be for primary care and again no insurance shall be billed.
Enrollment Fee
Membership Enrollment Fee $50 per person, $100 per household max if enrolled at same time.
Monthly Charges
Enrollment fee and first month of service is due at time of enrollment. Monthly charges will be billed as a recurring debit on the first of every month.
Pausing Payments
72 hour notice is required to pause payments.

Back to Basics Membership
$55 for adults 18 years & older $35 for children 0-17 years old

Membership includes:

Visits with provider for treatment of all preventative exams, chronic conditions, acute needs including hormone consultation. (Up to 30-minute appointment slot, same day or next day if needed. Weight loss not included.)
Returned provider calls within 24-48 hours.
Point of care labs including urinalysis, urine HCG, blood glucose testing, rapid flu, COVID, and strep testing.
Phlebotomy fee (additional discounted cost per lab test ordered, disclosed rate will be given prior to draw)
Intramuscular injection administration fee, (Medication cost separate and vary based on current cost to provider, all cost will be disclosed prior to administration.)
12 Lead EKG
Cutaneous procedures such as suturing and biopsy. (Pathology subject to lab cost)
Discounted prescriptions dispensed in clinic.(Excluding GLP or weight loss injections)
